I agree that rent is billed on a month‑to‑month basis and payments must be made on time each month. If a payment is late, a $30 late fee will be added for each day the payment remains unpaid. If I choose to vacate the property, I am required to provide a 15‑day written notice. If a 15‑day notice is not provided, rent will continue to be charged until the written notice is received.

I agree to Healthy Hearts Home Living’s lease requirements. I understand that all residents must sign required agreements and review the Policies and Procedures Handbook before move-in. Early termination fee and that noncompliance may result in fees, violations, or termination of residency.

I consent to the collection, processing, and secure encryption of my Personally Identifiable Information (including my income award letter, Social Security Number, and Government Photo ID) for housing application and background verification purposes as outlined in the Privacy Policy.
