Month-to-Month Residential Occupancy Agreement

I understand this is a month-to-month occupancy agreement that automatically renews each month unless properly terminated.*

I understand that the resident must provide at least 30 days’ written notice to terminate this agreement.*

I understand that if the resident vacates without proper 30-day written notice, the resident may remain responsible for one additional full month of rent.*

I understand that management may require immediate termination if house rules are violated, rent is not paid, or the resident’s level of care exceeds what can be safely managed.*

I understand that rent is due in full on the assigned due date and that there is no grace period.*

I understand that late fees may apply if rent is not paid in full by the assigned due date.*

I understand that automatic payment is the required payment method for rent and approved charges.*

I understand that AutoPay continues until proper written termination notice is provided and the notice period is complete.*

I understand that any required community fee is non-refundable.*

I understand that a risk fee may apply if background and/or credit screening is not completed.*

I understand that utilities are included up to the allowed monthly amount and that any overage may be split among residents.*

I understand that cleaning supplies and toiletries are provided once per month and that additional supplies may be the resident’s responsibility.*

I understand that residents must self-administer their own medications and that management does not administer or manage medications.*

I understand that Healthy Hearts Home Living is a shared living residence and not a licensed medical or skilled-nursing facility.*

I understand that payments made under this agreement are non-refundable, except where specifically allowed by management policy.*

I understand that video surveillance may be present in common areas only, and not in private bedrooms or bathrooms.*

I understand that management is not responsible for theft, loss, or damage to the resident’s personal belongings.*

I understand that management may conduct inspections with reasonable notice and may enter immediately during an emergency.*

I confirm that I have read, understand, and agree to the terms of this Month-to-Month Residential Occupancy Agreement, including automatic renewal, written notice, AutoPay, and immediate termination provisions.*
